Important Health Care Coverage Tax Documents are Available

The Vermont State Housing Authority is providing information about your 2025 health coverage, Form 1095-C, as required by the Affordable Care Act (ACA).

 

How to Obtain Your Form
While we are not mailing paper copies to everyone, your 2025 Form 1095-C is available upon request. To receive a copy, please contact us by:

 

Request Deadline
Please submit your request by March 31, 2026,  to ensure timely receipt for your tax filing.

 

This notice will remain in effect through October 15, 2026.
